NEW PATIENTS: We are not taking any new patients until OCTOBER, 2023 at the earliest and will not be responding to new patient inquiries until SEPTEMBER, 2023.
We have very limited availability for new patients. Due to this, we are only able to see new patients by referral. If you have been referred, complete and send the form to the right and we will contact you in September to schedule.
If you do not have a referral, we cannot see you at this time.
We will not respond to cold calls.
ESTABLISHED PATIENTS: Please refrain from email, and use the more secure Elation Passport for any non-urgent questions or concerns. If you have not seen Dr. Juniper in three or more years, you are considered a new patient. Please see the above.
