+ Discrimination policy

It is the policy of Dr. Juniper Martin's clinic to protect against discrimination based on gender identity, race, ethnicity, nationality, cultural background, sexual orientation, ability, or socio-economic circumstance.

+ Communication policy

Due to high volume, Dr. Martin responds to Elation messages as soon as possible, according to urgency. Patients are urged to contact Dr. Martin through Elation, not email, as it is more secure. If you have not received an invitation to Elation, and would like one, please request.

If you have a new health concern or extensive questions regarding your current treatment plan, it is best for your care to schedule an appointment. Dr. Martin cannot address new health concerns or extensive questions via email, Elation or phone.

If you have a simple question, please make your way to our FAQ (frequently asked questions) page, as it is likely answered there.

For prescription refills, please contact your pharmacy and ask for the appropriate refill request paperwork to be faxed directly to Dr. Martin's office at 503-443-2142. Please allow 3-4 business days for refill requests to be processed.

If you have a truly urgent issue to discuss with Dr. Martin, call or text 971-328-0071. Non-urgent calls and texts will be responded to within 2-3 business days.

If you are having a medical emergency, please call 911 or go to the Emergency Room.

+ Payment policy

If your insurance does not cover your visit for any reason, you are responsible for payment. Some reasons insurance might not cover: out of network provider, deductible not met, benefits maxed, pre-authorization required. It is not Dr. Martin or her staff’s responsibility to check benefits on your behalf, and you are expected to pay for your visit if your insurance does not.

Unsure of your benefits? Here’s how to check:

Call your insurance company using the phone number on the back of your insurance card. Give them the information as needed. They may ask for your member number, group number, name, birthdate, or social security number. This is all normal information to give them, so they can help you find your benefits. They will be able to give you information on what kinds of services are covered, what your deductible is, what your out-of-network benefits are, what your co-payment is, and more. To check if you are covered to see Dr. Martin, give them her full name and tell them she is a Primary Care Physician in Oregon.

Covered by Care Oregon, Oregon Health Plan, or Health Share of Oregon? As long as you are directly assigned to Dr. Martin, you will not have to pay for the cost of your visit, per state law. If you are NOT assigned to Dr. Martin, you may need a referral from your assigned doctor. If you have a question about your assignment, we can help.

+ No-shows and cancellations

No-Shows: Dr. Martin has a no-show fee of $50.00.

Cancellations: If you must cancel your appointment, do so prior to 24 hours before your appointment. If you cancel within 24 hours of your appointment, there is a $50.00 cancellation fee.

CareOregon/OHP Patients: These fees do not apply to you. However, we still appreciate receiving notice of cancellation earlier than later.
